Features & Capabilities
Care Plan. SmartPHRs display patients’ most recent continuity of care data in cross-provider, cross-condition care plan format, enabling providers with National Provider Identifiers to make permanent, source-identified, online entries. SmartEHRs give providers options for adding meaningful-use and quality-indicator decision supports and alerts to their patients’ care plans.
Clipboard. SmartPHRs display patients’ most recent continuity of care data in clipboard format, enabling patients and authorized friends and family to make online, permanent, source-identified entries. Patients automatically fax clipboards to providers when registering for visits, avoiding repetitive form completion.
Continuity of Care Data, CCD. The HL7/ASTM CCD includes structured data categories such as conditions, medications, and allergies. SmartPHRs display patients’ most recent CCDs to authorized users for export to USB key fobs or record banks or printing and storage in home and practice files.
Chronic care coordination. SmartPHRs and SmartEHRs give patients, families and health care providers tools for individualized, evidence-based, care plan implementation, monitoring and improvement.
Disease registry. SmartEHRs give authorized providers access to their patients’ continuity of care data in searchable cross-provider, cross-condition disease-registry format together with tools for evidence-based management of chronic conditions and documented adherence to relevant meaningful-use requirements.
HIPAA Privacy and Security Rules. Per the July 2010 Notice of Proposed Rulemaking from DHHS for 45 CFR Parts 160 and 164, Prosocial executes business associate contracts with all HIPAA-covered entities receiving data transmission, data access, or personal health record (PHR) services and provides breach notifications to all consumers who directly acquire PHR subscriptions.
Interoperable Personal Health Records, PHRs. Comprehensive SmartPHRs, per patient- and role-based authorization, automatically extract structured continuity of care data from provider electronic health records (EHRs) for consolidation with online data entries. Patients and authorized providers access up to date SmartPHRs anytime, anywhere from web-connected devices or EHR workstations.
Observations. SmartPHRs give patients online tools from web-connected devices and iPhones for collecting, graphically display and sharing data about symptoms, side-effects, adherence to provider medication and lifestyle recommendations and for visually inspecting the impact of interventions.
Privacy. For SmartPHR technology that enforces patients’ privacy preferences and manages their provider-specific and consent-specific consents to share information, Prosocial Applications is the proud winner of a challenge prize from the University of California, San Francisco Medical Center.
Protected health information, PHI. Authorized users may query SmartPHR and SmartEHR user interfaces for PHI-identified data (patients, clinicians) or PHI-free data (administrators, business associates, researchers). To avoid data re-identification, we do not export de-identified data files.
Security. SmartPHR and SmartEHR data are encrypted in transmission and storage so that if hackers breach system defenses, they encounter meaningless numbers.
